Position Summary
Curando Medical Center is seeking an experienced and dedicated Advanced Practice Registered Nurse with a strong background in HMO and Managed Care to join our growing clinical team. This position focuses on delivering high-quality, patient-centered primary care while meeting Medicare Advantage and HMO quality, preventive, and compliance standards.
Responsibilities
- Provide comprehensive primary care services to patients enrolled in HMO and Medicare Advantage plans
- Perform patient evaluations, diagnoses, treatment planning, and follow-up care
- Manage acute and chronic conditions including diabetes, hypertension, COPD, CHF, and related conditions
- Conduct Annual Wellness Visits (AWVs), preventive screenings, and health risk assessments
- Document all clinical encounters accurately and timely in the Electronic Health Record (EHR)
- Coordinate referrals, prior authorizations, and continuity of care with specialists
- Collaborate with physicians, medical assistants, care coordinators, and administrative staff
- Follow Curando Medical Center clinical protocols and evidence-based guidelines
- Participate in quality initiatives including HEDIS measures, RAF coding, and risk adjustment programs
- Educate patients and families on care plans, preventive care, and medication adherence
Requirements
- Active Florida APRN / Nurse Practitioner license
- Current national certification (AANP or ANCC)
- Minimum 2 years of experience in HMO, Medicare Advantage, or Managed Care environments preferred
- Strong knowledge of value-based care models, HEDIS, RAF, and AWVs
- Proficiency with EHR systems and clinical documentation
- Ability to manage a structured patient panel efficiently
- Strong communication and teamwork skills
- Bilingual (English/Spanish) strongly preferred
